Why Does Liver Disease Cause Leg Swelling and Muscle Cramps? Signs You Must Not Overlook

1. Why Does Liver Disease Cause Symptoms in the Legs?

The liver functions as the body’s metabolic center. When liver function deteriorates, this impairs albumin production (causing water retention), glycogen storage (forcing consumption of essential amino acids), and detoxification. All of these can produce visible symptoms in the legs.

The Cause of Leg Swelling (Edema): Albumin Deficiency

The liver produces albumin, a protein that maintains the balance of water in the blood. When liver function declines, albumin production decreases. With reduced albumin, the ability to hold water inside blood vessels weakens, causing water to seep into surrounding tissues. This is called “hepatic edema,” and the lower limbs are particularly prone to it due to gravity.

The Cause of Muscle Cramps: Amino Acid Metabolism Abnormalities and Nerve Hypersensitivity

When the liver’s function to store glycogen (sugar) is impaired, the body is forced to break down muscle to generate energy, consuming essential amino acids such as BCAA and taurine. Depletion of these amino acids impairs normal muscle contraction and nerve function, leading to nocturnal cramps.

2. A Checklist for Distinguishing Ordinary Fatigue from Liver Disease

The following symptoms warrant prompt medical consultation:

• Indentation remaining on the leg after pressing with a finger (pitting edema)

• Yellowing of the whites of the eyes or skin (jaundice)

• Red palms (palmar erythema)

• Abdominal swelling (ascites)

• Persistent fatigue and weakness

• Nocturnal muscle cramps (especially in the calves)

3. The Importance of Proper Diagnosis by a Hepatology Specialist

Leg swelling and muscle cramps can have many causes—heart disease, kidney disease, venous thrombosis, and more. Accurate diagnosis through blood tests (albumin, bilirubin, liver function tests), abdominal ultrasound, and specialized nutritional management is essential to identify whether the cause is liver-related.

4. A New Approach for Advanced Cirrhosis: Regenerative Medicine

For patients with advanced cirrhosis causing persistent leg swelling and muscle cramps, our clinic offers stem cell therapy using adipose-derived cells. By promoting liver cell regeneration and reducing inflammation and fibrosis, this therapy can help improve albumin production and overall liver function, potentially alleviating these troublesome symptoms.

5. Summary: Do Not Ignore Signs in Your Legs

Leg swelling and muscle cramps can be signals that your liver is struggling. Early consultation with a hepatology specialist is crucial. Don’t dismiss these symptoms as ordinary fatigue—your liver may be sending you an important message.

At Saito Clinic of Internal Medicine, we offer comprehensive liver assessment including blood tests, abdominal ultrasound, elastography, and consultation for regenerative medicine. Both in-person and online consultations are available throughout Japan.
