Liver and Biliary System

Cirrhosis, hepatitis (hepatitis B, C, autoimmune), primary biliary cholangitis (PBC), biliary calculus cholangitis, cholangiocarcinoma—these cause itching through accumulation of bile acids. In primary biliary cholangitis particularly, itching often appears as the first symptom, making diagnosis prone to delay.

Expert Insight: Refractory Diseases and Cholestatic Itching

Numerous complex cases of PBC combined with autoimmune hepatitis (AIH) have been diagnosed and treated, with reports published in English-language medical journals. In these diseases, “cholestasis”—impaired bile flow—tends to occur from early stages, manifesting as stubborn whole-body itching that dermatologists cannot explain. Difficult-to-treat autoimmune liver diseases can lurk behind symptoms dismissed as “just dry skin.”

Kidneys, Thyroid, Blood Diseases, Diabetes

CKD and dialysis patients experience itching from uremic toxin accumulation in skin (“uremic pruritus”). Hyperthyroidism causes whole-body itching from increased skin blood flow. • White nails (Terry’s nails): most of the nails turning white and cloudy, seen in chronic liver disease with low albumin
• Clubbing: fingertips enlarging into drumstick shape, seen in advanced liver and respiratory diseases
• Cutaneous telangiectasia: fine blood vessels becoming visible through skin, can indicate progression of cirrhosis

3．Self-Check Sheet: Confirming Whether Itching Is Coming from Internal Organs

• Over-the-counter anti-itch cream/moisturizer provides no improvement
• Itching becomes particularly intense at night (before and after sleep)
• Strong itching despite normal skin appearance
• Whites of eyes or skin appear yellowish (jaundice)
• Urine is dark (beer-colored, orange)
• Fatigue, loss of appetite, or weight loss present
• Sense of heaviness or discomfort at upper right of abdomen (liver location)
• Told at health check that ALT, AST, or γ-GTP is high
• Diagnosed as hepatitis B or C carrier
